An ID card for seniors with dementia.

 

This is the front of the Safe Return card.

The Safe Return card by the National Council of Social Service (NCSS) is an ID card that is generated through the Developmental Disability Registry (DDR) for the safety and protection of seniors with dementia. The Safe Return card provides a source of identification for seniors with dementia when they are lost.

DDR is an electronic system used by Voluntary Welfare Organisations (VWOs) to effectively manage client’s information and monitor client’s development over time. It also aids VWOs in their collaborative efforts in the provision of coordinated services to clients. The card details include name and photo, date of birth, gender, VWO contact and contact number of the caregiver, as well as optional information like residential address, language spoken by and special needs of the cardholder.

In situations where seniors with dementia are unable to inform the authorities of the contact details of their caregivers, the information printed on the card will come in handy. The VWO contact on the card will also enable the police to enlist their help. Other professionals, such as medical personnel, will also know whom to contact in an emergency based on the information on the card. Safe Return cardholders can also enjoy discounted rates for services and access to facilities offered by participating companies listed on the NCSS website.

Caregivers of seniors with dementia who are already clients of participating VWOs (www.ncss.org.sg/social_service/SRcard_listing.asp) can apply for the Safe Return card at their respective VWOs. Alternatively, caregivers can download a copy of the application form from the Alzheimer’s Disease Association (ADA)’s website, or contact the ADA Caregiver Support Centre via the Dementia Helpline at 6377 0700 for more information.

 

 

The first Safe Return card is issued free of charge. Subsequent replacement cards issued may be chargeable at S$2 each.
